If you are over the age of 18, you will need to bring a VALID GOVERNMENT PHOTO ID.
If you are under the age of 18 then you need to be accompanied by a legal parent or guardian. You will also need a photo ID (see list of acceptable ID) and your parent or legal guardian must also bring their VALID GOVERNMENT PHOTO ID and/or LEGAL GUARDIAN PAPERWORK.

If your parent or guardian do not have matching last names then a Birth Certificate is also required.
Parent/Guardian must be present at all times whilst procedure is being done.
We do not pierce anyone under the age of 5yrs.
ACCEPTABLE PHOTO ID
When under the age of 18 and being accompanied by a parent/legal guardian who has valid government photo ID, the following are acceptable.
Yearbook photo with name underneath
Basically anything which has your photo with your name next to it.
To avoid being refused we suggest that you bring the birth certificate along with you also.
